Title: Partnering for Prevention with Community Health Workers

Southern Michigan Diabetes Outreach Network (SODON) has established unique partnerships for training community health workers. This partnership has integrated a unique blend of professional and consumer roles that extends to and beyond the medical community. Community health workers reach minorities such as the under or non-insured, disparate groups and low as well as middle income individuals. These unique partnerships incorporate diabetes prevention and diabetes complications prevention into their communities. This public health approach to reach individuals, who do not or infrequently access the healthcare system, results in providing healthcare support, social support, and initiates empowerment skills towards improved diabetes self-management.
