Title:  Increasing Access to Preventative Care in Rural Areas

Abstract :
Primary prevention strategies aimed at persons without diabetes should be encouraged to get regular medical exams and learn how to maintain their health.  Secondary prevention strategies for persons with diabetes include regular medical exams and specialty referrals as needed.  Specific tests should be included and metabolic goals included.

TENDON Diabetes Outreach Network built their strategic plan recommendations in conjunction with the Michigan diabetes strategic plan and Recommendation 3 stipulates that we look for solutions for disparities in accessing prevention services in the rural areas.  A work group was formed and represents Allegan, Barry, Montcalm and Kent Counties.

The work group discussed problems in the rural area, then set the following goal:  educate the rural family practice physicians on the early recognition of kidney disease and referral.  A kidney brochure was updated by the Kidney work group and the Rural work group used this tool.  Laminated posters delineating some of the Standards of Care will be done upon grant request and approval of monies in the near future.

Evaluation of increasing annual testing of microalbumin found an increase in the awareness to do this test from all four sites (Allegan General Hospital, Montcalm Diabetes Outpatient Program, Pennock Hospital, and United Lifestyles).

Future goals include looking at lipids, neuropathy and vascular assessment.
