Title: Michigan Diabetes Outreach Network Diabetes Care Improvement Project

Objective: To assure that persons with diabetes receive care according to the clinical practice recommendations.

Methods:  A schematic diagram illustrates the steps involved in the Michigan Diabetes Outreach Network Diabetes Care Improvement Project.
Initial visit and follow-up data are collected to determine improvement in care. Scannable forms and electronic data entry are options used for collecting data. Participating agencies collaboratively problem solve to work on areas needing improvement.

Results: Results in Michigan have demonstrated improved outcomes. Trend in follow-up data from FY 1996 through FY 2001 for glycosylated hemoglobin, foot exams, and microalbuminuria show significant improvement in number of persons having these tests done.

Conclusions: A regional network of collaborating agencies can improve diabetes care through use of a coordinated diabetes care improvement program and working together to problem solve with creative strategies.
