Title: Show Me the Money

Abstract: A large, rural region in Michigan with a high and increasing rate of diabetes complications received a $40,000 grant to address diabetes issues. A large part of the initial intervention included implementation of diabetes care assessment with regular follow-up and referral of home health patients with diabetes. Home care staff also received enhanced diabetes care training. Follow-up by a state employed researcher showed a lower rates of diabetes related hospitalization and amputations in agencies using tracking system. Program expanded throughout state with state tobacco tax money and CDC funding– annual budget $1.8 million in 2002 plus support from the state Diabetes Control Program. Program expanded in scope also – diabetes tracking in a variety of settings, more professional education, including self paced modules, assessment of community diabetes needs enhancing services where few exist and working on community awareness of diabetes issues. Tracking and showing favorable outcomes and keeping legislators informed and involved helps sustain funding.
